FEE AGREEMENT CONTRACT

1. Have CLIENT send for his/her file from each of the three (3) major credit reporting
agencies to review his/her credit files in order to evaluate the problems and develop the
best course of action for correcting the errors therein for CLIENT, as spelled out in the
AGREEMENT CONTRACT, paragraph number 1.
2. FEES FOR THIS SERVICE IS $2,500.00
3. $1,000.00 down with the signing of this AGREEMENT.
4. $500.00 monthly payments for the following three (3) months.
5. MONTHLY PAYMENTS ARE DUE IN OUR OFFICES ON OR BEFORE THE 5TH OF EACH MONTH.
6. PAYMENTS ARE TO BE CHECK OR MONEY ORDER PAYABLE TO: ASK CONSUMERS CREDIT SERVIES,
INC.
The parties hereto have duly executed this FEE AGREEMENT CONTACT on this ______ day of
_____________, 199 ___.
*Timely payments means that the payment has to be in our office on or before the due date
and in the agreed amount. There is a late fee of $45.00 per month. This fee is placed on
the account after the 7th day of each month when the payment is late.
